ALL HUMAN NETWORK SYSTEMS (HNS) PROFESSIONAL ARE MEMBERS OF AGING LIFE CARE ASSOCIATION (ALCA), PREVIOUSLY KNOWN AS NATIONAL ASSOCIATION OF PROFESSIONAL GERIATRIC CARE MANAGERS (NAPGCM). ALL ARE ALSO CERTIFIED CARE MANAGERS (CMC) OR CERTIFIED CASE MANAGERS (CCM). THE THREE CO-OWNERS ARE ALSO NATIONAL CERTIFIED GUARDIANS (NCG) AND MEMBERS OF THE NATIONAL GUARDIANSHIP ASSOCIATION (NGA) AND THE COLORADO GUARDIANSHIP ASSOCIATION (CGA).
Director: Gordon P. Wolfe, MSW, LCSW, NCG, CMC. Founder of HNS in 1984 and currently Co-Owner, Gordon is a pioneer in the fields of Geriatric Care Management and Private Guardianship services. Over the years has been active on several national and regional boards, conferences, author, and speaker at local, regional and national events for attorneys, guardians, and care managers. Current HNS involvement part-time as consultant, supervisor, and administrative duties with some direct client contact.
Director: Chris Reifenrath, PT, MA Gerontology, CMC, NCG. Chris became Co-Owner of HNS in 2010; joined the agency in 2002 after 14 years’ experience as a physical therapist in the Lutheran Hospital system. Chris specializes in complex cases with multiple medical issues as well as helping families cope with grief, loss, and end-of-life issues. Certified in 2008 in Contemplative End-of-Life Care (CEOLC). Chris has been active in national and regional NAPGCM activities, was the Western Regional GCM Chapter President in 2010 and 2011, and has presented at regional and local conferences for attorneys, care managers, and guardians. In 2015 she received the Western Regional GCM/ALCA Chapter Rose Kleiner Award for outstanding dedication, leadership, and service. Chris has routinely served as Guardian or Co-Guardian of individuals since becoming Co-Owner of HNS in 2010.
Director: Erin Custer Dougher, MA Gerontology (Long-Term Care Administration), CMC, NCG, RCFE. Erin is an expert in long-term care options and became Co-Owner of HNS in the summer of 2015. Post-master’s experience includes nursing home, social services and assisted living administration. Erin joined HNS in 2006 after several years as Health Services Director at Heritage Club Denver (now Brookdale University Park). She has been on various planning committees for NAPGCM conferences and local professional organizations. Fluent in Spanish, Erin specializes in situations with complex psycho social dynamics and multiple providers involved. Named as Co-Guardian in cases since 2016.
Care Manager: Jennifer Pagano, MA Gerontology, CMC. A Denver native with 15 years’ Care Management experience, Jennifer joined HNS in 2007 after 7 years’ at Open Care in Charlotte, N.C., a geriatric care agency. Jennifer’s specialty is dementia care training for family and professional caregivers. Jennifer also worked at theAlzheimer’s Association in Charlotte, N.C., and helped develop a specialized Alzheimer’s adult day program there. She was active in the Southeast Chapter of NAPGCM and was named their Outstanding Member in 2005. Since returning to Colorado, Jennifer has been active in Western Regional GCM activities, including conferences, committee work, and a conference presentation. Jennifer was instrumental in developing and leading the former Colorado Local Unit of NAPGCM.
Care Manager: Cheryl Hampton, MA, CRC, CCM. Cheryl has a B.A. in Psychology, a Master’s degree in Counseling, and is a Certified Rehabilitation Counselor and a Certified Case Manager. She joined HNS in 2014 and currently covers some of HNS’ most difficult cases. Her prior work included experience with developmentally disabled adults, substance abuse, domestic violence counseling, mental health, and family therapy for child protective services. She is skilled at writing comprehensive long term needs assessments and has experience working with Personal Injury (PI) attorneys developing life care plans for individuals with catastrophic injuries or illnesses.
Care Manager: Erin Taylor, MSW, LSW, CMC. Erin joined HNS in 2015 after nine years’ experience as a lead social worker at Innovage, the well-known regional PACE provider for the elderly. Prior work experience includes work in two local hospitals, a mental health clinic and the Alzheimer’s Association. Before coming to Colorado in 2003 Erin worked in nursing homes in her native state of Pennsylvania. With her past excellent experience, Erin is a solid member of the HNS staff.
Care Manager: Weronika Dorman, MSW, LSW, CMC. Born in Poland, Weronika grew up in Chicago and is fluent in both Polish and English. Prior post-Master’s work experience was with Adult Protective Services in Chicago and with a Care Management agency similar to HNS in both Chicago and Denver. Now firmly transplanted to Denver, Weronika joined HNS in mid-2016 and is a strong contributor to the HNS team.
